The Power of Three: Collaboration Can Help Stroke Survivors Return to Independence

Although stroke-related deaths have decreased substantially in the last few decades, research suggests that acute ischemic stroke hospitalizations rates have increased, as well as stroke risk factors and the prevalence of people who have three to four risk factors.

With preventive wellness health services including nursing and hospice, and various myriad services such as respite care, and through the delivery of meals—these three agencies are seeking an opportunity to support stroke survivors by accelerating their return to independence following a hospital stay.
