As more community-based organizations explore better ways to address health-related social needs through cross-sector partnerships, Community Care Hubs (CCHs or hubs) are emerging as contracting and network leads across many communities. This catalog is a compilation of resources that address readiness for becoming a Community Care Hub, considerations for network formation and operation, and the important role that hubs provide as the contracting and administrative lead for community-based networks of social service providers. This catalog provides direct links to resources to support existing and emerging hubs in expanding their capacity to contract with health care entities to eliminate health disparities and inequities in access to care.
