Network Member Checklist: Considerations for CBOs Deciding to Join a Network

New health care policies and quality measures are creating opportunities for the emergence of Community Care Hubs (CCHs or hubs) across the country. These hubs play an integral role in streamlining access to coordinated care through cross-sector partnerships at the local, regional, and state level. As community-based organizations (CBOs) explore the potential to join networks of social service providers led by these hubs, there are important questions to consider. This Member Checklist is intended to support CBOs in understanding how local hubs operate and manage their networks. Its series of questions to consider is designed to help inform CBOs about important qualities and practices to assess viable opportunities to join one of these networks, and what would be required of their individual CBO to successfully perform as a network member.
