New Tools to Help You Build a Stronger Network

Increasingly, health care providers and payers are seeking to contract for home and community-based services through a single point of contact. These new opportunities have sparked the growth of community-based integrated care networks (CBICNs, or networks). To help CBOs improve the foundation of their networks, the Aging and Disability Business Institute has developed a new set of resources to help guide networks as they assess and strengthen core operations for health care contracting.

Through six new interactive worksheets, CBO networks will answer questions to identify action steps that will improve operations for health care contracting. These steps can then be used to develop an action plan on the accompanying Network Operations Action Plan Template. Together, these worksheets will serve as a launch pad for discussion and the development of plans that will strengthen your network’s operations. We suggest that all members of your network’s leadership team contribute to these worksheets together.

Your network may wish to supplement this work by using the Business Institute’s Network Readiness Assessment, which helps networks evaluate their current level of readiness for building, sustaining and growing a coordinated network of service providers for health care contracting.

These worksheets were created for participants of the 2020 Network Operations Learning Collaborative, which was funded by the U.S. Administration for Community Living, U.S. Department of Health and Human Services through a contract with the Lewin Group. Points of view or opinions do not, therefore, necessarily represent official Administration for Community Living policy.
