The Social Determinants of Health: Key Factors in Creating Value Through CBO-Health Care Partnerships

 

The increasing prevalence of value-based arrangements places providers at risk for not only cost, but also quality and outcomes. This movement from paying for activity to paying for results has forced providers in value-based arrangements to carefully assess cost, access, care delivery and coordination throughout the care continuum. With some estimates suggesting psychosocial factors and personal choices account for 70 percent of health outcomes, this outsized impact means social determinants of health need to be taken into account across the care continuum, and multiple parties need to understand their role in an older adult’s health.

But traditional healthcare providers and community-based organizations (CBO) that address social determinants of health have previously had little incentive to collaborate for a variety of reasons, including misaligned payment incentives and cultural and language barriers. While a number of efforts to link healthcare to broader social needs are emerging through delivery and payment initiatives, these efforts are nascent and remain fraught with obstacles.
