A Right-Minded Partnership Focuses on Healing in Community

This Aging and Disability Business Institute article highlights the enduring relationship between Senior’s Resource Center (SRC) and the Lutheran Medical Center. These Denver, Colorado. based centers initiated a pilot partnership that sought out to help older adults suffering from chronic heart failure, avoid hospital readmissions, and to transition successfully back to their communities. The pilot program gave way to the “A Perfect Homecoming” initiative focusing on three main aims including, reduced risk of hospital readmissions, improved overall quality of life, and cost-savings to the community.

The 30-year SRC-Lutheran partnership has made evident strides in reducing hospital readmissions since its launch, and credits constant communication, collaboration, positive intent, and a sharing of a trust relationship, as contributors to their withstanding success.
