The Centers for Medicare & Medicaid Services’ (CMS) new health related social needs (HRSN) screening—at the hospital inpatient level and with physicians—as well as social determinants of health (SDOH) data and technology standards are providing Area Agencies on Aging (AAAs), and community-based organizations (CBOs) with opportunities to expand partnerships with health clinicians to better serve clients and caregivers.

The first webinar in this series will focus on new CMS reporting measures related to screening for health-related social needs that will impact (and likely increase) referrals for assessments and services from hospitals and health systems to social care organizations. Speakers will discuss what the rules mean for AAAs, CBOs and networks, how they create new opportunities for contracting with health care providers and payers, and what CBOs need to know to work with hospitals and health systems.

This webinar series will provide information about the new opportunities to partner with health clinicians, and the related data, information exchange and related billing/payment models being advanced by CMS. Look out for other webinars in our series that will cover topics such as:
