SHARP Function Checklist: Decision Points for CBOs Considering Working with Social Health Access Referral Platforms

Social Health Access Referral Platforms (SHARPs) are a new type of technology platform that are increasingly being used by health care providers and payers to refer patients to community-based organizations for services to address health-related social needs. A new tool from the Aging and Disability Business Institute and Partners in Care Foundation helps Area Agencies on Aging and other CBOs that are considering or already working with SHARPs.
