Final Report: Western New York (WNY) Integrated Care Collaborative Community-Based Integrated Care Networks

In recent years, policy makers have changed how they think about health care and long-term services and supports. This shift is changing expectations of what care looks like and how providers are paid. Consequently, new thinking has brought about new opportunities for the aging network, but to capitalize on them, Area Agencies on Aging and other community-based organizations must embrace a new way of thinking as well.

This report describes the efforts of what has come to be called the Western New York Integrated Care Collaborative and the work it has done to explore new ways of organizing the aging network to meet changing service delivery expectations. It includes history, lessons learned, and recommendations for future network development.
