In partnership with Scripps Gerontology Center at Miami University, we learned from our first Request for Information Survey (RFI) in 2017, that aging and disability community-based organizations (CBOs) across the country are partnering with health care payers and providers to address social and behavioral determinants of health. This report served as a preliminary look into the landscape of contracting between community-based organizations and health care entities. A second RFI report in 2018 yielded further insight on how contracts had evolved over the previous year.
