You may not know it, but your colleagues in the aging and disability networks are innovating their way towards robust health care partnerships. Community-based organizations (CBOs) across the geographic and service spectrum are positioning themselves for integrated care success with a variety of health care payers and providers.
This Aging Today supplement features four organizations that have been selected to participate in a Trailblazers Learning Collaborative (TLC), convened by the National Association of Area Agencies on Aging (n4a) and funded by the Administration for Community Living (ACL). They are the “leading-edge” of the Aging and Disability Networks, the first group of community-based organizations (CBOs) to tackle next-generation CBO–health care partnership issues. The collective will serve as a “think tank” for prototyping, and work together toward solutions to address challenges and opportunities in contracting with health care entities.

- Senior Nutrition Services, a division of New Opportunities, Inc. (Waterbury, CT)
- Ability360 (Phoenix, AZ)
- University of New Hampshire’s Institute on Disability (IOD)
