A Collaboration Among Primary Care-Based Clinical Pharmacists and Community-Based Health Coaches

A Collaboration Among Primary Care–Based Clinical Pharmacists and Community‐Based Health Coaches examines the impacts of managing patient medication through a partnership between health coaches and pharmacists. Findings from this study suggest that a home visit conducted by a community-based health coach combined with a medication review by a primary care-based pharmacist may prevent hospital readmission in older adults recently discharged from the inpatient care.
