NCOA Launches 2018-2019 Learning Collaboratives

The NCOA Center for Healthy Aging is pleased to announce the kick-off of both 2018-2019 Learning Collaboratives. The LCs will provide support to either: 1) develop network hub partnerships with community-based organizations (CBOs), or 2) pursue Medicare Part B reimbursement for the following benefits: Diabetes Self-Management Training (DSMT), Health and Behavior Assessment and Intervention (HBAI), and Chronic Care Management (CCM).

If you are interested in either option, please complete the associated application (links below) by COB on Monday, March 19, 2018.  Learning Collaborative charters and applications have been included as attachments for your review.  Please be thoughtful before responding about your interest, as applicants and their partners must be willing to provide appropriate time and resources from their organizations to actively participate in either learning collaborative (see “Participant Expectations” in each charter). Organizations should coordinate with others in their designated coverage area or state and identify one organization as the lead on this learning collaborative initiative.
