This October 19th, 2016 webinar, developed by n4a and the Aging and Disability Business Institute and hosted by the ASA, Area Agencies on Aging (AAA) and hospital leaders  describe how their partnership evolved to create common goals and initiatives on two levels: population health, and individual patient health. Initiatives range from data collection and trend analysis, to co-sponsored projects and interventions, to shared patient plans of care and exploration of payment models for sustainability. A survey process for assessing population health resulting in collectively owned follow-up strategies is presented as well.
