Feorene, MBA, health care consultant, will highlight the importance of the social determinants of health (SDOH) and discuss how Accountable Care Organizations and health systems can collaborate with community-based organizations (CBOs) to ensure access to this vital segment of the care continuum.
Traditional health care providers and community-based organizations (CBO) that address SDOH have previously had little incentive to collaborate for a variety of reasons, including misaligned payment incentives and cultural and language barriers. While a number of efforts to link health care to broader social needs are emerging through delivery and payment initiatives, these efforts are nascent and remain fraught with obstacles. Participants on this webinar will:
- Understand the vital importance of addressing social determinants of health as part of the care continuum to achieve high value outcomes;
- Discuss the respective needs, drivers and contributions of the health systems and CBOs; and,
- Recognize effective partnership characteristics through review of a case study.
