Medicare enrollees with chronic conditions and functional limitations represent more than half of Medicare’s highest spenders, and are more likely to use hospital inpatient and emergency department services. How can you meet the health and long-term services and supports needs of your health plan members with chronic conditions and functional limitations, and keep them living at home, where they prefer to be? Community-based aging and disability organizations can serve as your “eyes and ears” in the home and not only provide needed services, but also engage members in evidence-based health programs that can help them manage their own health.
This webinar, created for the Aging and Disability Business Institute and hosted by the American Society on Aging (ASA), features an innovative partnership between a health plan and a community-based organization helping to improve health outcomes and quality of life for older adults and persons with disabilities.
