Medicare enrollees with chronic conditions and functional limitations represent more than half of Medicare’s highest spenders, and are more likely to use hospital inpatient and emergency department services. Community-based aging and disability organizations can serve as your “eyes and ears” in the home and not only provide needed services, but also engage members in evidence-based health programs that can help them manage their own health.
