How can you meet the health and long-term services and supports needs of your health plan members with chronic conditions and functional limitations, and keep them living at home, where they prefer to be? Community-based aging and disability organizations can serve as your “eyes and ears” in the home and not only provide needed services, but also engage members in evidence-based health programs that can help them manage their own health.
