Many of the over 30 million Americans living with diabetes and 84 million with prediabetes face challenges in managing their conditions. State and community-based organizations across the country have built the capacity to implement and scale evidence-based programs like the Diabetes Prevention Program and Diabetes Self-Management Program to support individuals in managing complex health needs. However, many programs are supported by limited funding and need to develop sustainability plans for the future. Join this webinar to hear strategies and lessons learned from 4 organizations in Virginia that have explored various methods to expand and sustain programs long-term.
