This webinar will introduce participants to the Return on Investment (ROI) Calculator for Partnerships to Address the Social Determinants of Health, which was developed with support from the Commonwealth Fund. Health care organizations and community-based organizations (CBOs) have limited tools to support joint planning, goal-setting and estimating the financial return on investment, sustainability and health outcomes of new pilots or CBO programs that focus on addressing the social needs of complex patients. The ROI Calculator can be an important tool to help organizations collectively address these issues to build better partnerships by helping each partner to become more ready for the work of collaboration.

AJ Diamantopoulos is the Accountable Health Communities manager for the Denver Regional Council of Governments (DRCOG) Area Agency on Aging. He leads the team that works with clinical and community providers to demonstrate the critical link between better health outcomes and increased access to community-based services. Before joining DRCOG in 2013, Diamantopoulos worked as a small business owner and as an advocate for victims of domestic violence, followed by a role as a service coordinator with older adults for Volunteers of America and in Durango, Co.
Karol Tapias joined n4a in March 2019 as the deputy director of the Aging and Disability Business Institute. Karol came to n4a from LiveOn NY, where she spent 12 years in multiple roles, most recently as the associate executive director.
