2019 ASA Aging in America Conference to Feature Managed Care Summit Focused on Chronic Condition Management

As part of the 2019 Aging in America Conference, the American Society on Aging (ASA), in partnership with the Aging and Disability Business Institute, will host a Managed Care Summit focused on chronic condition management.

New ways of paying for value-based care have changed the landscape for delivering services to older adults and people with disabilities. The health care sector now recognizes the role of community-based organizations (CBOs) in integrating long-term services to improve the quality of life for individuals.  Partnerships between the sectors are resulting in cost savings for the health care sector and sustainability for CBOs as they leverage their expertise and diversify their revenue streams. However, these partnerships require transformation of how both sectors approach the delivery of care.  This program focuses on managing multiple chronic conditions from the payer and policy perspectives.  Each participant will receive a copy of the Generations Journal Field Guide containing seventeen articles on this topic that delve deeper in the issues of community-based care.

Objectives:

•     Review the complexities of managing multiple chronic conditions in older adults and the potential for enhanced quality of life, if managed well;

•     Identify the complexities that social determinants bring to the equation when managing the health of older adults;

•     Recognize the enhanced role community-based organizations bring to health care sector in helping older adults with complex needs do well in community;

•     Discuss the role Medicare, Medicare Advantage, and the CHRONIC Care Act affect care and the competition for payer reimbursement.
