The latest publication from the Aging and Disability Business Institute, Building Collaborative Contracts with Health Care: Western New York Integrated Care Collaborative and Independent Health, highlights the work of The John A. Hartford Foundation 2023 Business Innovation Award winner, Western New York Integrated Care Collaborative (WNYICC) for its work as a Community Care Hub to address the social determinants of health. Through its 25 contracts with health plans, WNYICC’s more than 50 members help reduce barriers older adults and people with disabilities may face in accessing medical care by addressing the health-related social needs of individuals living in western New York through a variety of programs.
