Partnering with Primary Care to Improve Access and Care Coordination

 As primary care practices, operations, and structures undergo transitions throughout the health care environment, new models now exist that open up space for area agencies on aging (AAAs) and other community-based organizations (CBOs) to become real, funded partners in primary care transformation.

This presentation, part of part of n4a’s 2017 Aging Policy Briefing, breaks down the changing primary care landscape, offering insights into the novel care and payment systems that CBOs can provide value to, including patient-centered medical homes, accountable care organizations, and newly expanded chronic care management codes for Medicare.
